Dear Mr. Aumentado:
4LEAF, Inc. (4LEAF) appreciates the opportunity to submit our Cost Proposal to provide Construction
Management (CM) and Inspection services for the City of Cupertino De Anza Blvd and McClellan Road /
Pacifica Dr. Intersection Modification (“City”) Public Works Project (“Project”). This proposal supersedes
our proposal dated August 23, 2023, to include costs for our sub consultant Contractor Compliance
Monitoring, Inc. (CCMI) to perform contractor labor compliance monitoring services.
The attached Cost Proposal provides a detailed breakdown of our estimated level of effort and includes
the following general assumptions:
One part-time Construction Manager at an average of 2 hours per day week during the Project
which is estimated by the City at 80 working days.
The contractor’s work schedule was not available at the time of preparation of this estimate
but based on conversations with you, the general allowable work hours are assumed to be 9 am
to 3 pm (6 hrs/day) for work performed in the intersection and along De Anza and may be
expanded to 8 hrs/day for work along McClellan and Pacifica Drive. For cost estimating purposes,
we have assumed one Construction Inspector at 6 hours/day for 40 working days for work in the
intersection and along De Anza and 8 hrs/day for 40 working days for work along McClellan and
Pacifica Drive.
Includes budget for our subconsultant, Construction Testing Services (CTS), to perform Special
Inspection and Testing services for the Project.
Includes budget for our subconsultant, CCMI to perform contractor labor compliance monitoring
services. Based on previous and current projects we are utilizing CCMI’s services, we have
assumed a budgetary estimate of $2,000 / month for five months (assumes construction phase
is 80 working days or four months plus one month following construction during project
closeout).
Our cost proposal does not include budget for detailed or in-depth labor compliance monitoring
services. If necessary, we can request a written cost proposal for these services from our
subconsultant, Contractor Compliance & Monitoring (CCMI). 4LEAF staff will provide
intermittent and cursory reviews of contractor and subcontractor certified payroll information
that is provided with the contractor pay applications.

The Project is Federally funded, and City staff will be responsible for completing and maintaining
all federal-funding reimbursement forms.
We understand that the City has awarded the general contracting contract to Redgwick Construction and
the Project is tentatively scheduled to begin construction in November 2023. No baseline schedule from
the contractor was available at the time of preparation of this proposal. Because the exact project start
date is currently unknown, we cannot identify the specific staff who will be assigned to the Project if
awarded to 4LEAF. As a result, we are providing the resumes for several staff who may potentially be
assigned to the Project (see Attachment B).

(a) Assumes project will require compliance with California Prevailing Wage rate requirements under SB 854, and the Client will be required to file a PWC-100 Form to the California Department of Industrial Relations for the project.
(b) Per the requirements being enforced under SB 854 and because it is assumed that a PWC-100 Form will be filed by the City to the CA DIR, 4LEAF is required to notify an authorized Apprenticeship Committee through submittal of a DAS-140
form. We are then required to make an official request to an authorized Apprenticeship Committee for an apprentice by submitting a DAS-142 form. We are not assured the apprenticeship committee will be able to provide a suitable /
qualified apprentice for the project. Per the apprenticeship requirements, the hours worked by the apprentice must be in a ratio of 1:5 for apprentice to journeyman hours. Hours are included as a placeholder in the event that an
apprenticeship for a public works inspector is required and dispatched by the apprenticeship committee. We have assumed the public works apprentice assigned to the project will be classified as a Period 1 Apprentice as defined by the DIR's
Wage Determination dated 8/22. In the event that a higher classification apprentice is assigned to the project, we will contact the City to request a change order for this line item.
(c) 4LEAF staff will be supplied with typical tools of the trade to perform their daily tasks (e.g. cell phone, laptop computer, and PPE).
(f) Per information provided by Marlon Aumentado with the City to Gene Barry with 4LEAF, the City anticipates 80 working days (120 calendar days or 16 weeks) to complete the project and the contractor's work hours will generally be 9 am to
3 pm (6 hrs) for work on De Anza but may be extended to 8 hrs/day for work on McClellan and Pacifica Drive. For estimating purposes, we have assumed one, part-time Construction Manager at 2 hours per day during the construction phase.
We also assume one Construction Inspector for 40 Working Days at 6 hours per day at regular-time rate for work on De Anza and 8 hrs per day at regular-time rate when the contractor works 8 hrs/day. Does not include any allowance for
overtime or any nighttime, weekend, or holiday work. If overtime, nighttime, weekend, or holiday work is required, the requirements for these hours will be billed in accordance with California Prevailing Wage Law requirements. If construction
duration extends past 80 working days or the contractor works more than 8 hours per day during the project, 4LEAF will submit a budget amendment request.
(d) Not intended as a Constructibility Review but for 4LEAF staff to come up to speed on the project.
(e) Assumes the City will provide budget for the Project Design Team to remain engaged and available during the pre-construction and construction phase to provide responses to design clarificatons, RFIs, Submittals, etc.

BASIS OF CHARGES
GENERAL
Fees for tests and inspection include cost of technician, normal equipment and regular reports. Engineering services will be charged at applicable rates and will require travel and
mileage charges for equipment transport and storage per code (portal to portal) from the nearest CTS laboratory. Soils testing with nuclear gauge and/or sand cone equipment and
inspections requiring equipment will require applicable travel and mileage charges for equipment transport and storage per code (portal-to-portal) from the nearest CTS laboratory.
Fees for special projects, services overseas, or elsewhere in the United States, will be quoted on request. With prior notification to Client; charges are subject to change at any
time. Construction Testing Services reserves the right to adjust the rates quoted in this contract based upon any Union or prevailing wage increases and/or changes in any industry
requirements.
MINIMUM HOURLY CHARGES – INSPECTION
Technician personnel and the following minimum charges are contractual commitment:

TESTS
Testing fees shown include normal time for performing test. Samples requiring special preparation will be charged at the laboratory technician rate. Fees for tests not listed will be
quoted upon request. There will be a minimum charge of $100.00 for any engineering report. Please note some tests maybe tested by subconsultants. Samples delivered to the
laboratory after 3:00pm or samples needing results within 24 hours will incur a 50% mark-up.
INSURANCE
The liability of Construction Testing Services (CTS) is limited to CTS's contract value.
PAYMENT
Invoices will be submitted monthly or bimonthly for services performed during the preceding month and are payable on receipt. Interest of 1.5% per month (but not exceeding the
maximum rate allowable by law) will be payable on any amounts not paid within 30 days, payment thereafter to be applied first to accrued interest and then to the principle unpaid
amount. Attorney’s fees or other costs incurred in collecting any delinquent amount shall be paid by client. Visa, MasterCard and American Express payments are accepted
however fees will apply. Visa and MasterCard payments require an additional 3% on top of the amount of the invoice being paid. American Express payments require an additional
4% on top of the amount of the invoice being paid.

Chris was 4LEAF’s Construction Manager for these two City projects. The two adjacent projects were
constructed concurrently by two different contractors. The Basketball project included performing lime
treatment of court subgrade, court and adjacent walkway excavations, asbestos-containing pipe (ACP)
remediation, paving and placement of concrete flatwork, storm drainage system modifications,
installation of 10 ft. deep reinforced hoop footings, acrylic surfacing of the court, and landscape
restoration (hydroseeding and irrigation adjustments). The Community Gardens project included
installation of a new irrigation system, subgrade placement of geotextile fabric rodent entry, construction
of new corrugated metal panel and wire mesh fencing, installation of 14 new planter boxes and two ADA-
compliant planter boxes, site appurtenances (Tuff Shed, benches, garden table), vehicular and pedestrian
concrete flatwork, AC paving, and landscaping.

Construction Representative
Chris performed construction inspections and construction administration duties on behalf of 4LEAF for
this $5.5M project. The project included constructing alterations to the shoreline and beach area facilities;
enhancing shoreline protection features; constructing flood protection features; creating a larger beach
area; demolishing existing restrooms and constructing new facilities; installing new storm drain catch
basins and pipelines, new water lines, and irrigation piping; constructing concrete curb and gutters,
performing cement treatment and stabilization of on-site soils, paving new parking lots, installing on-site
detention basins, installing landscaping, and performing reconstruction of the eastern promenade area.
Chris was responsible for providing daily inspections and documenting job-related activities; monitoring
and documenting the contractor’s work for adherence to contract plans and specifications; coordinating
with San Mateo Parks and Recreation staff; monitoring the contractor’s storm water pollution prevention
protection BMPs; coordinating 4LEAF’s subconsultant, CTS, for special inspections and materials testing;
reporting all discrepancies requiring corrective actions to County of San Mateo staff; reviewing contractor
monthly pay requests and make recommendations for payment; coordinating with San Mateo County
Parks biologist for periodic inspections, ensuring contractor’s compliance during bird breeding/nesting

North Shoreview Flood Improvements Project

Chris performed construction inspections on behalf of 4LEAF for this $24M project that is being
constructed to allow for more than 2,000 homeowners in surrounding neighborhoods to apply for a
Federal Emergency Management Agency (FEMA) flood insurance exemption. The project includes
constructing a temporary detour segment of the Bay Trail around the project locations and through the
North Shoreview neighborhood; upgrading electrical controls, correcting structural deficiencies, installing

Balboa Way Repair Project

Drive and Arguello Blvd. The improvements consisted of restoration of the entire extent of the residential
area of Balboa Way between Anza Drive and Arguello Boulevard as well as private driveways. The project
included the replacement / repair of approximately 27,300 square feet (SF) of roadway asphalt,
replacement of approximately 5,170 SF of sidewalk including curb and gutter, replacement of 10 private
driveways, construction of four ADA curb ramps, replacement of approximately 630 lineal feet (LF) of 8‐
inch‐diameter high density polyethylene (HDPE) sanitary sewer pipe (SDR 17) by open trench,
replacement of approximately 155 LF of 24‐inch HDPE sanitary sewer pipe (SDR 17) by open trench,
abandonment and relocation of approximately 720 LF of a 6‐inch‐diameter ductile iron water main line,
replacement of approximately 90 LF of 18‐inch reinforced concrete pipe (RCP) storm drain line with 18‐
inch HDPE (SDR 17) pipe by open trench, construction of three new sanitary sewer manholes, leak repair
using grout injection and interior coating, pressure testing of seven existing sanitary sewer manholes, and
re‐establishing sanitary sewer lateral tie ins. Included installation of fire hydrants, and approximately 20
new water meters. Several water mains were connected using a ‘hot tap’ method and all potable water
lines were disinfected and BAC‐T testing conducted by 3rd party prior to use. Included removal and
disposal of transite (asbestos containing ) pipe that was previous used for potable water.

Road and Stevens Creek Bridge. Work included traffic control; removal of existing vegetation in medians
and roadway shoulders, griding and removal of the upper 1 ½ inches of asphaltic concrete and replacing
with cold‐in‐place (CIR) asphalt; removing and resetting utility covers to match final grade; placing
shoulder backing along edge of pavement; replacing traffic inductor loops; miscellaneous repairs to
driveway entrances and installing traffic striping.
City of Pacifica Serra Drive Storm Drain Outfall Repair Project

Chad was 4LEAF’s inspector who performed construction inspections and biological survey construction
monitoring for this project that included the demolition and removal of the existing concrete spillway,
headwall, and HDPE storm drainpipe; construction of a coffer dam to divert flows around the construction
site; slope grading; construction of seven new reinforced concrete piers, concrete spillway, headwall, and
wing walls; installation of new rip rap and new partially grouted rip rap energy dissipator; and site
restoration. The project was completed along San Pedro Creek which is a perennial stream that flows
through Pacifica. The project was constructed in conformance with U.S. Fish & Wildlife Streambed
Alteration Agreement requirements. Existing fish or wildlife resources identified as potentially being
adversely impacted included the California red‐legged frog, the San Francisco dusky‐footed woodrat,
Steelhead – Central CA Coast DPS, the Western red bat, and nesting birds.
City of San Mateo North Shoreview Flood Improvements Project

Chad performed construction inspections for this $24M project that is being constructed to allow for more
than 2,000 homeowners in surrounding neighborhoods to apply for a Federal Emergency Management
Agency (FEMA) flood insurance exemption. The project includes constructing a temporary detour
segment of the Bay Trail around the project locations and through the North Shoreview neighborhood;
upgrading electrical controls, correcting structural deficiencies, installing backup power generation,
installing new pump station outfalls, and increasing pumping capacities at the Coyote Point and Poplar

1325 Old County Road Water Line Improvements Project

District (MPWD) for the developer-funded Artisan Crossing Building being constructed in the City of
Belmont and San Mateo County. Components inspected include water mains, water laterals, and new fire
hydrants being installed in the roadways [Old County Road (OCR), Elmer Street, Karen Road, and Oneill
Ave.] surrounding the perimeter of the Artisan Crossing Building. Mary was responsible for ensuring the
water service improvements were constructed per the City of Belmont and MPWD’s standards, as well as
the approved project plans.
OCR is a major utility corridor and numerous transmission gas mains, storm drain lines, water mains and
laterals, sanitary sewer lines, and telecommunication and fiber optic trunk lines are located within the
project corridor in addition to distribution mains and overhead electric. OCR also includes heavy vehicular
traffic and is lined with multiple businesses, which requires extensive traffic control and community
outreach.

Alvarado-Niles Pipeline Seismic Improvement Project
Senior Construction Inspector
Marty was 4LEAF’s Senior Inspector for this $12M waterline replacement project along Smith Street and
Alvarado-Niles Road in the City of Union City. Phase 1 of this project consisted of furnishing and
installing approximately 3.5 miles of 14-inch, 16-inch, and 18-inch diameter welded steel pipeline, as
well as installing service laterals and meter boxes, air vacuum valves and assemblies, and a cathodic
protection system. The scope of work also included performing all the associated pressure and leakage
testing, disinfection, and installation of system connections (closures) to the existing water mains.
The project required close coordination with multiple stakeholders and permitting agencies including
City of Union City (Union City), Union Sanitary District (USD), Pacific Gas & Electric (PG&E), California
Department of Transportation (Caltrans), Alameda County Flood Control & Water Conservation District
(ACFC&WCD) and Union Pacific Railroad (UPRR). The project was located along a heavily travelled
roadway consisting of a downtown business district, private residences, an elementary school, and
crosses both UPRR and I-880.
Marty’s responsibilities included performing extensive public outreach and coordination with local

Downtown Monterey Road Rehabilitation Project

Marty was 4LEAF’s Senior Inspector for this $1.6 million federally-funded roadway rehabilitation project
on Monterey Road between 1st St. and 8th St. The improvements consisted of pavement rehabilitation
and roadway reconstruction; microsurfacing and digout pavement repair; construction of missing and/or
damaged concrete curbs and gutters; and the installation of ADA compliant curb ramps at numerous
intersections within the project limits. Monterey Road is a heavily travelled roadway lined with businesses
and restaurants and required an extensive amount of public outreach to coordinate restaurant deliveries,
drop off and pick-up for two schools that were located one block off Monterey Road, etc. Marty ensured
the business owners on Monterey Road and residents that lived on the surrounding streets received
current construction schedules and were notified of potentially disruptive construction activities. 1st St.
is a Caltrans-operated roadway, so coordination was required with Caltrans to obtain a double permit for
the work near 1st St. As a federally-funded project, Marty and 4LEAF’s Construction Manager ensured
the project met all the requirements outlined in the Caltrans Local Assistance Procedures Manual (LAPM).
Alameda County
Capital Improvement Projects
Construction Inspector
Under 4LEAF’s On-Call Inspection contract with Alameda County, Marty provided inspection services for
multiple public works improvement projects including the Davis Street Pump Station Project and the
Laguna Creek SB6-149 Improvement Project.
The Davis Street Pump Station Rehabilitation Project included removing and replacing a damaged sump
pump and removing debris and sediment within a confined space structure. Marty performed inspection
duties including preparing daily reports, providing photo documentation, and tracking line items for
monthly pay estimates. He also verified all the personnel entering the confined space had certificates of
completion in confined space training.
The Laguna Creek SB6-149 Improvements included constructing a retaining wall along Laguna Creek
between South Grimmer Dr. to Highway 880. Marty inspected the restriping of the northbound and
southbound number 5 lanes on Highway 880 at Laguna Creek, as well as the installation of 2,000 l.f. of
K-rail in preparation for future work. Marty worked with Caltrans to secure a permit to complete the
related work.

the installation of a 42-inch welded steel waterline encased in concrete. Marty was responsible for
ensuring permits were secured prior to construction. Marty inspected the backfill and compaction,
scheduled testing with the City’s lab, and verified the waterline installation complied with the project
specifications. Coordination with Caltrans was required, because a portion of the waterline was within the
Caltrans right-of-way. The waterline was installed in an extremely congested area with high traffic
volumes, and extensive public outreach with residential neighborhoods and local businesses was
required.

Zanker Road Major Interceptor Phase I and II Project

Prior to joining 4LEAF, Marty inspected the installation of an 84-inch RCP sanitary sewer line using jack-
and-bore methods under existing twin Hetch Hetchy water mains. The construction of the first structure
was 32 feet deep and 22 feet below sea level. Groundwater pumping and dewatering was needed to
stabilize the area prior to pouring the foundation. This structure had 16 chambers and eight openings with
four 84-inch major inlet interceptors and four outlet interceptors. The twin 84-inch sewer mains, going
from structure A to structure B, were installed by a straddle crane 50 feet wide and 70 feet in length. This
major operation required trucks to deliver a single pipe as the straddle crane picked from the truck and
stabbed into place with a bulldozer making grade and stockpiling excess soils. This major interceptor
installation required removing existing utilities and replacing with new communications, water lines,
water services, power lines, storm drains, sewer mains sewer lateral utilities and services. This included
surface infrastructure asphalt roadway curb and gutter sidewalk and a row of trees beyond the right of
way on the private property of the Sony building.

Sanitary Sewer Improvement Project

Prior to joining 4LEAF, Marty was the Construction Inspector for this project which included multiple
sewer diversions from 84 to 12 inches. This included two major sewer spills and cleanup. Bypass pumping
of sanitary sewer for cured and place products, such as Instuform and Uform/Reform PVC Liner. From
steam cure to boiler units hot water circulation cookout, most of these were 72 hours around the clock
processes. Marty inspected the rebuild of the entire infrastructure on Bird Avenue in Willow Glen, a highly
sensitive neighborhood. The project removed and replaced all utilities and services; removed and replaced
sidewalks and wheelchair ramps to meet the new ADA standards; and removed an entire roadway section
including curb, gutter, and sidewalk.

Senior Construction Inspector
Prior to joining 4LEAF, Marty monitored and inspected work for contractor’ compliance with plans,
specifications, and schedules and prepared project documentation including daily reports, force accounts,
materials testing, and other construction records. He was responsible for conducting planning,
preconstruction, coordination, progress, scheduling, and field staff meetings; reviewing plans,
specifications, submittals, shop drawings, change orders, constructability reviews and contract closeout
record; assuring operations, measurements and inspections were scheduled; ordered and satisfactorily
completed and documented for quality assurance/quality control compliance. Marty verified and prepared
quantity and progress estimates for progress payments; maintained records on unit price quantities, labor,
equipment and progress for change order work; and reviewed and approved change orders for
implementation. Marty also made field measurements and maintained as-built drawings records. Marty is
well versed in Caltrans policies and procedures.
