TO HUMAN SERVICE GRANT FUNDS
CITY/NON-PROFIT CONTRACT
The First Amendment to Contract is entered into between Support Network for Battered Women
(CORPORATION) and the City of Cupertino
(CITY) to amend the Contract entered into pursuant to the allocation and disbursement of
Human Service Grant funds to Corporation dated Apr 15, 2008 (CONTRACT).
WHEREAS, the CITY has reserved a portion of i is General Fun to be used on activities that
benefit Cupertino's residents; and
WHEREAS, the City Council authorized the City Manager execute amendments with
organizations as needed to implement the FY 2009/10 allocation of Human Service Grant
funds; and
WHEREAS, the parties wish to enter into this amendment to extend the term of the
CONTRACT, revise the Project Description, Implementation Time Schedule and Project
Budget;
THEREFORE, the parties agree as follows:
1. Section I, entitled "Project," is revised to read as follows:

3. Supplement Exhibit A, the Project Description, Exhibit B, the Performance
Measurements, Exhibit C, Proposed Implementation Time Schedule, and Exhibit D,
Project Budget, with Revised Exhibits "A through D," attached hereto and
incorporated herein by this reference.
4. All terms, covenants and conditions stated in the CONTRACT, which are not herein
amended, remain in full force and effect.

Project Description:
Please describe the project with as many details as possible. Include general information, such
as the number of units, number of Cupertino residents that will be served, services that will be
provided, etc.
The proposed project will provide comprehensive; domestic violence services to 14 Cupertino
residents experiencing or surviving domestic violence at a cost per client of $300. Services
provided include access to a toll-free 24-hour bilingual (Spanish) crisis hotline, emergency
shelter, group and individual counseling, legal assistance, social advocacy and community
outreach/education. Abreakdown of the number; of unduplicated Cupertino beneficiaries at or
below current countywide median income includes: 12 at 0-30%, 1 at 31-50%, and 1 at 51-80%.
Specifically, Support Network will provide coun.~eling services to 10 unduplicated Cupertino
residents, and will provide emergency shelter services to four (4) unduplicated Cupertino
residents. Counseling services include information and referral, crisis intervention, individual
and group counseling, long-term therapy, case management, legal advocacy and court
accompaniment, parenting classes (operated in accordance with county protocols), and
therapeutic and support services for the children of abused women. Support Network offers
bilingual Spanish/English services to Spanish-speaking residents.
With respect to emergency shelter services, clients seeking safe haven may stay in our shelter
with their children for up to eight (8) weeks. During their stay, they will receive case
management services to increase self-sufficiency and independence, along with assistance in
locating employment if necessary, and transitional or permanent housing. If at any time the
shelter is at capacity, clients are assisted in locating an alternate shelter location.
EXHIBIT A
Desired outcomes for the Domestic Violence Services for Cupertino Families Project include:
• Sixty five (65) percent of clients completing at least 15 weeks of individual therapy or
one 10-week cycle ofpsycho-educational group counseling; will increase their Domestic
Violence Self Efficacy Score by 25%.
• Seventy five (75) percent of Cupertino residents on leaving the emergency shelter will
have an improved understanding of personal safety issues and a more stable living
situation.
