09-097 Emergency Housing Consortuim (EHC Life Builders)FIRST AMENDMENT TO HUMAN SERVICE GRANT FUNDS CTTY/NON-PROFIT CONTRACT The First Amendment to Contract is entered into between Emergency Housing Consortium (EHC Life Builders) (CORPORATION) and the City of Cupertino (CITY) to amend the Contract entered into pursuant to the allocation and disbursement of Human Service Grant funds to Corporation dated Apr 15, 2008 (CONTRACT). WHEREAS, the CITY has reserved a portion of its General Fu>a{to be used on activities that benefit Cupertino's residents; and WHEREAS, the City Council authorized the Cit•~ Manager execute amendments with organizations as needed to implement the FY 201)9/10 allocation of Human Service Grant funds; and WHEREAS, the parties wish to enter into this amendment to extend the term of the CONTRACT, revise the Project Description, Implementation Time Schedule and Project Budget; THEREFORE, the parties agree as follows: 1. Section I, entitled "Project," is revised to read as follows: "CITY agrees to allocate a portion of its current Human Service Grant budget to CORPORATION, being the sum of ThreE~ Thousand Nine Hundred Eighty Seven Dollars and No Cents ($3,987.00). for the purpose of implementing the CORPORATION'S project, as more particularly described in numerous exhibits as noted Herein, attached to this First Amendment. 2. Section II, entitled "Term," Parts A & B :ire revised to provide a contract end date of June 30, 2010. 3. Supplement Exhibit A, the Project Description, Exhibit B, the Performance Measurements, Exhibit C, Proposed Implementation Time Schedule, and Exhibit D, Project Budget, with Revised Exhibits "A, through D," attached hereto and incorporated herein by this reference. 4. Program participants include homeless families with children, single adults, and unaccompanied homeless and runaway youth. 95% are extremely low income, earning less than 30% of Area Median Income. Cupertino residents will have access to EHC LifeBuilders' lull Continuum of Care, including emergency shelter, transitional housing programs, and comprehensive supportive services. Supportive services include basic needs such as food and clothing, as well as case management, housing location assistance, employment training, referrals, and assistance accessing public services. The Project also provides Cupertino residents with a €;ateway to EHC LifeBuilders' array of services beyond emergency shelter and transitional housir.~g. *A PSD is EHC LifeBuilders' unit of service, representing a single day/night of shelter and accompanying services for one person.
