TO COMMUNITY DEVELOPMENT BLOCK GRANT
CITY/PUBLIC SERVICE CONTRACT
The First Amendment to Contract is entered into between Senior Adults Legal Assistance
(CORPORATION) and the City of Cupertino
(CITY) to amend the Contract entered into pursuant to the Catalog of Federal Domestic
Assistance for allocation and disbursement of Ccminunity Development Block Grant
(CDBG) funds to Corporation dated Apr 15, 2008 (CONTRACT).
WHEREAS, the CITY has applied for and recei~ ed the FY 2009 funding allocation for
CDBG funds from the United States Department of Housing and Urban Development; and
WHEREAS, the City Council authorized the Citr~ Manager execute amendments with
organizations as needed to implement the FY 2009 allocation of CDBG funds; and
WHEREAS, the parties wish to enter into this amendment to increase CORPORATION's
CDBG entitlement, extend the term of the CONTRACT, revise the Project Description,
Implementation Time Schedule and Project Budget;
THEREFORE, the parties agree as follows:
1. Section I, entitled "Project," is revised to :read as follows:
"CITY agrees to allocate a portion of its current CDBG entitlement, as defined in 24
CFR 570 Subpart J, "Grant Administration," to CORPORATION as a subrecipient
for eligible public service activities, in the; suin of Nine Thousand Seven Hundred
Seventeen Dollars and No Cents ($9,717.00).
for the purpose of implementing the CORPORATION'S project, as more particularly
described in numerous exhibits as noted herein, attached to this First Amendment.
2. Section II, entitled "Term," Parts A & B are revised to provide a contract end date of
June 30, 2010.
3. Supplement Exhibit A, the Project Descri~~tion, Exhibit B, the Performance
Measurements, Exhibit C, Proposed Implementation Time Schedule, and Exhibit D,
Project Budget, with Revised Exhibits "A through D," attached hereto and
incorporated herein by this reference.
4. All terms, covenants and conditions stated. in the CONTRACT, which are not herein
amended, remain in full force and effect.

Project Description
Please describe the project with as many details as possible. Include general information, such as the
number of units, number of Cupertino residents that will be served, services that will be provided, etc.
SALA is anon-profit law office that provides free legal services to Santa Clara County residents
that are age 60 or older, with an emphasis upon those who are low income or in social need. SALA's
staff of 5 attorneys has expertise in problems common to our target population. SALA's legal staff is
augmented by 10 volunteer intake workers who assi:;t with client interviews at senior centers and by
15+ pro bono attorneys who provide Simple Wills through SALA's No Fee Wills Panel. Legal
services (in the form of advice/referrals, consultations, representation, and community education) are
provided in the following areas: Public Benefits (So~;ial Security, SSI, Medicare, Medi-Cal, In Home
Supportive Services); Elder Abuse; Nursing Homes; Advance Health Care Directives; Consumer;
Housing; and Simple Wills.
For 2009-2010, SALA will maintain our expanded service levels at the Cupertino Senior Center
by being available for appointment sessions at the frequency of at least two times a month. SALA will
also make home visits to Cupertino elders who are homebound or institutionalized. Clients with
urgent legal problems will be served on an "emergency" basis by telephone. Cupertino elders who
appear at SALA intake sites in other cities will also be served.
As detailed in the Project Work Plan, SALA will provide direct legal assistance to a minimum of
40 Cupertino clients in 2009-2010. This goal includes all elders served through SALA's Cupertino
project as described above and not just those assisted through the expansion at the Cupertino Senior
Center. SALA also proposes to conduct one commu~iity education presentation in Cupertino.
