I regret to inform you that your application for a massage therapist permit has
been denied. This denial was due to your not passing the massage examination
administered by Roth Consulting, The City of Cupertino is required to deny
your application based on the requirements listed in chapter 9,06.120 of the
Cupertino Municipal Code,
Cupertino Municipal Code section 9.06.120 (Massage Therapist Practical
Examination) reads in part as follows: "The applicant shall take and pass a
written test and practical performance examination administered through a
medical practitioner approved by the City of Cupertino, wherein the applicant
has been required to demonstrate a basic knowledge of anatomy, physiology,
hygiene and massage."
You do have the right to appeal this denial decision by writing a letter to the
City of Cupertino City Clerk's office within five business days in receipt of this
letter. 1 have enclosed a copy of the appeal form that must be filled out, along
with your appeal letter and all documents must be mailed or hand delivered to
the City Clerk's office. The City does have a $150.00 appeal charge that will be
refunded, if the appeal is granted. The appeal will be heard before the
Cupertino City Council at their regularly scheduled meeting.
