Kaiser Permanente is requesting a formal change of address for the property currently
listed as 10950 Taman Ave. The building was leased in late 2002, and is slated for use as
a mental health outpatient facility, with some administration functions housed at the site.
Half of the building is currently occupied, with the other half scheduled for occupancy in
late 2005. Physically, the structure is one building, currently with two street addresses
(10950 Tantau Ave, and 19000 Homestead Road -please see attached site plan). The
• existence of two addresses is causing confusion for our members, who expect to find one
building with one address. Based on the nature of our members' visits to;this site, the
more simple the wayfinding, the better they are served. It is with this benefit to our
members in mind that this request is made.
