Item 1OF: REGARDING ITEM #10A FROM REPORT #27693.
FINDING: As indicated on our previous report we found an excessive moisture
condition and fungus damage at the upstairs hall bathroom floor covering
underlayment, subfloor and wall plate.
RECONIvIENDATION: At the owner's request the following repairs are being
outlined. We will remove the existing toilet. Remove the complete existing
vinyl floor covering and floor underlayment to expose the damaged subfloor
and wall plate. We will remove the damaged subfloor and wall plate as
necesary and check for concealed damage. If no further damage is found new
subfloor and wall plate materials will be replaced/reinforced as needed. The
same toilet will be reset on a new wax seal, properly bolted. Our bid does
not include new floor underlayment or new vinyl floor covering. If any
further damage is found another Supplemental Report will be issued with our
additional findings and cost.

FINDING: As indicated on our previous report we found fungus damage to the
subfloor and wall plate at the upstairs hall bathroom tubback area. The
damage extends under the bathtub.
RECONN'IENDATION: At the owner's request the following-, repairs are being
outlined. We will remove the complete existing.rtubba&',Vto\ include the

ceramic the and sheetrock. We will also remove and dispose of the existing
bathtub. The fungus damaged subfloor andk�ki 'plate will be removed and
replaced with new materials (in conjunction with item #10F) . We will then
install new sheetrock at the tubback walls, taping and sealing all voids. No
texturing and no painting are included. Our bid does not include the
installation of a new bathtub or new tile at the tubback walls. If any
further damage is found another Supplemental Report will be issued with our
additional findings and cost.

FINDING: On our previous report we outlined to remove and replace the
upstairs hall bathroom tub/shower fixture.
RECONE4ENDATION: At the owner's request the following repairs are being
outlined. we will remove the existing tub/shower fixture and install a new
anti-scald guard fixture, properly plumbed. The owner is going to purchase
and provide the fixture that we will install. This must be a standard
tub/shower fixture, and not an unusual or difficult fixture to install (not
a Eurcpean,fixture; . If the selected fixture requires additional plumbing
repairs additional costs will be incurred. Please call our office if you
have any questions or if you would like to discuss the type of fixture that
has been selected.

REC'OMME 110N: After trades have set the bathtub we will return to the
property and install the new sheetrock and also reconnect the tub drain,
using a new waste and overflow kit. The cost for these repairs is included
with item #10G.

Item 1OK: FINDING: At the owner's request the following bid is being provided.
RECOMMENATION: Remove the existing upstairs hall bathroom tile vanity top
and backing. We will then prep this area as necessary. Other trades are
going to install a new vanity top and a new sink. The owner is to provide a
new standard type fixture. After trades have completed their repairs, we
will return to the property and reconnect the new sink and new fixture. We
will provide and install new supply valves, new supply lines, etc. Our bid
does not include a new vanity top, a new sink or a new fixture. NOTE: The
new sink/vanity top must line up properly with the wall plumbing. If any
extraordinary plumbing repairs are required, additional costs will be
incurred.
