2. Only the LAST DIGIT will be considered.
3. The odd/even addressing system will be maintained.
4. The change of address will not result in a public safety hazard.
5. PROOF OF OWNERSHIP IS REQUIRED. (Ex. property tax bill)
The fee for a change of address request is $245.00. The fee is due with this
request form and will not be refunded if the request is denied.
The direct costs associated with an address change request will be borne by the
applicant. Approximate review time is fifteen (15) days. If the address change is
granted, the new address will be in effect thirty (30) days following approval.

Dear Greg:
This letter is to request a change of address for the Dynasty Restaurant at Vallco Fashion
Park.
You will find attached a Vallee, lower level lease plan showing store numbers assigned to
• the lower level tenants. The new Dynasty space is currently going into the space marked
1044. However, the owner of the restaurant has requested that the space be renumbered to
1688.
Typically, the management would not honor such a request but due to the fact that the
restaurant is located at the end of the numbers assigned to the spaces on the lower level, it
would not impose any complication for us at the mall level.
Therefore, we are requesting that the building department review this change and let us
know of your decision. Please keep in mind that the entire mall's address is 10123 N.
Wolfe Road, with each of the mall tenants having their own space number.

City of Cupertino Request for Change of Address
All change of address request forms shall be submitted to the City of Cupertino
Building Department for approval. The following criteria must be met:
1. The change of address will not create confusion.
2. Only the LAST DIGIT of the address will be considered
3. The odd/even addressing system will be maintained.
4. The change of address will not result in a public safety hazard.
5. Proof of ownership (Ex. property tax bill) is required. In the case of a commercial
tenant requesting a change of address, written agreement from the property
owner must be presented at the time of application.
• 6. All commercial buildings having a single address assigned with multi-suite
arrangements shall have a numeric suite-numbering system approved or
assigned by the Building Department: Ex. ABC Systems
