REPORT OF TESTING AND OBSERVATION SERVICES
DURING PAD GRADING OPERATIONS
References: 1) Geotechnical Investigation
By: T. Makdissy Consulting,Inc.
Dated: Feb 4,2011
Dear Mr. Koontz:
In accordance with your authorization T. Makdissy Consulting, Inc, has provided engineering
consultation, observation and testing services during pad grading operations for Lot 2 building pad
of the subject project. This letter is a summary of our observations and test results.
Grading on the subject lots commenced by stripping, clearing the area of all existing vegetation and
debris and removing 18 to 24 inches from the building site. The native material was then scarified,
moisture conditioned, then compacted to a minimum relative compaction of 90%. Fill material
consisting of quarry fines was imported and placed on the building pads to bring it to design grade.

The satisfactory placemenbof fill was determihed by field observation and performance of in-place
field density and laboratory compaction t'e'sts:;The laboratory compaction tests were performed to
determine the maximum dry'.density:and the optimum moisture content of the soil used for fill.
i
Compaction tests.were:.performed in.accordance with Laboratory Test Procedures ASTM D1557-
94. The results ofthese tests are sumniarized'on Tablet-I. A nuclear gauge was used to determine
the in-situ percentage=of dry,:density, and;moisturc content. Nuclear gauge density methods were
generally in accordance.,withAS,'IM.D2922-91.and D3017-88. The in place test results along with
the achieved relative compaction are included in:Tables I & Il.
Pad grading operafions, for the building-pad were generally performed in accordance with the
recommendations^contained in the, aforementioned reference. The pad is ready to receive building
improvements. Surfacedrainage must be-provided as designed by the project Civil Engineer and
maintained by the property'owners at all'timM The building pads are ready to receive foundation
improvements.
