Before scheduling inspections please fill out, sign and return the enclosed Information/Authorization Form. Also, please attach the city approved Special Inspection and "testing Schedule and a copy of the permit, if available. Without the returned In ormation/Authorizaeion Fermis reports cannot be processed and sent to the city, resulting in possible delays to your project. 2. Testing Engineers issues reports for a project based on its permit number. On a project with multiple permits we need to track the work performed for each one. Let us know of all permits issued for your project. If revised permits are obtained, inform our office and our inspector at the work site immediately. Failure to do so will result in inaccurate progress reports, Final Report delays, and the possibility that we may not be able to include the revised permit number on the Final Report. 3. Schedule inspections as far in advance as possible. During busy periods inspections may need to be scheduled up to 72 hours in advance to guarantee an inspector. With the exception of periodic welding inspection, the code requires you to have an inspector on your work site whenever work requiring special inspection is being performed. 4. For welding inspection, our inspector needs to be present at the start of any welding, even when periodic visual inspection is permitted. Review of approved plans, welding procedure specifications, welder qualifications and fit -up needs to be done by our inspector prior to any welding. If the welding has been completed prior to inspection, it will be noted in the progress reports and as an exception in the Final Report, possibly creating problems for you with the city. 5. For concrete inspection, our inspector needs sufficient time to inspect the rebar and anchor bolt placement prior to concrete placement. Rebar placement cannot be performed while the concrete is being placed. if our inspector does not have sufficient tirne to inspect the rebar, it will be noted in the progress report and as an exception in the Final Report, possibly creating problems for you with the city. 6. For proof -load testing, our inspector needs the proof -load value and rate of testing provided by the responsible engineer (if not already on the approved Drawings). Without this information, our inspector cannot perform the tests. 7. When scheduling inspections be as thorough as possible when describing what inspections you will need. This will insure that an inspector with the proper qualifications is sent to your project with all of the appropriate equipment. 8, if you require an inspector on an ongoing basis give our dispatcher a conservative estimate of the duration. If an inspector is scheduled for a 4=hour inspection don't assume their availability beyond that point. In most cases they have been scheduled for a later inspection. 9. * The Final Report must be requested, for each permit number, after all the required special inspections have been completed. Allow at least 3 working days for the Final Report to be processed. Remember that concrete and masonry samples are tested 28 days after sampling.
