Gary Stapleton reported to Jeremy with XL Construction at the jobsite. Reviewed welder qualification papers for
David Boaz, ID #1262993. Observed welding in progress completed by 1 welder using FCAW process with E71T-8
NR 232 wire. Weldments observed consisted of single pass fillet welds joining angle to WF beam top flange and
stiffener plates to WF beam for glass handrail support at column line D -E/6.5, 2nd floor.
Welded 6" leg of support angles were trimmed approximately 5/8" because of existing field condition. To
make up 1 1/4" of weld on both sides of angle, fillet weld size was increased from '/4 to 3/8" fillet weld and fill
weld across heel of angle to beam footing. Pending engineer approval. Punch list cleared 10/6/12 per
engineer sketches SK 2, 3, and 4 dated 9/27/12.
Work observed was, to the best of Gary's knowledge, in conformance with approved plans, specifications and
applicable standards of workmanship, except as noted.

Kevin Meyers reported to Jeremy with XL Construction at the jobsite. Reviewed welder qualification papers for
David Boaz, ID #1262993. Observed welding performed and completed by (1) welder previously reported using
FCAW process with E71T-8, NR232 .072" diameter wire. Weldments observed consisted of single pass fillet welds
joining angle to beam and plate to beam for guardrail supports along W27 girder, 2nd Floor along D-139/2 per detail
D-4/2 (shop drawing A/E6). Note: This is a follow-up inspection to inspection performed by Gary Stapleton on
10/6/12.
Work observed was, to the best of Kevin's knowledge, in conformance with approved plans, specifications and
applicable standards of workmanship.

Carlos Ramirez — ID #6618 and David Boaz — ID #5203. Observed welding performed and completed by Lee's
Imperial welders using FCAW process with E7010 electrodes and SMAW process with E71T-8 wires. Weldments
observed consisted of partial penetration, single pass fillet welds, and arc spot welding at lines D -D.4/4 2nd level
joining tube to channel and tube to angle per SKI; D.4/4-6 2nd level joining angle to angle per SK2, D -D.4/6 2nd
level joining tube to angle truss (5/S7), top plate to angle, plate to WF beam; plate to plate at lines D/4, D.1/4, D.4/4,
D/6, D.1/6, and D.4/6 (shop plans need RFI), existing 6x6 angle to angle truss, 3x3 angle to existing angle and
angle ledger (verbal from engineer need RFI); D.4/4, D.4/6 2nd level plate to WF beam, plate to deck flutes, angle
kicker to WF beam, and angle ledger at lines D/4-6 2nd level. In progress welding of decking to framing members.
Exception: Need RFI for following items: top plate to plate, angle WF beam, partial joint penetration weld at
lines D/4, D.1/4, DAM, D/6, D.1/6, D.4/6 called out in shop plans but not structural; and existing 6x6 angle to
angle truss, 3x3 angle to angle ledger and existing angle. Received verbal from engineer during walk through
at lines DAM and D.4/6. See attached non-compliance report. Acceptable members were marked with
identification.
