OWNER'S ACKNOWLEDGMENT AND VERIFICATION OF INFORMATION

Lti1. I understand a frequent practice of unlicensed persons is to have the property owner obtain an "Owner -
Builder" building permit that erroneously implies that the property owner is providing his or her own labor and material

provide coverage for those injuries. I am willfully acting as an Owner - Builder and am aware of the limits of my

Dear Patricia,
I would like to request for an extension for the building permit application # 14030163.
Site Address : 11560 Lake Spring Ct, Cupertino
Current Expiration date: 11/11/2014
The permit was for remodeling a master bath and guest bath. The guest bath was completed and inspected.
However, due to unforeseen scheduling delays with our contractor, delays in getting tiles etc. our master bath
work has gotten delayed. The project has started but will not be ready for inspection by 11/11/2014. I would
like the date to be extended by 180 days.
Appreciate your consideration in granting us the extension. Please let me know if you need any further
