I have been a client with the Cupertino Case Management program for three years, and the
program has enabled me to continue to live independently and safely in my own home. Over
the course of three years, the case manager has assisted me to transition from a Skilled Nursing
Facility back to my home, identified and secured a dependable caregiver who helps with my
daily living needs, and advocates on my behalf regarding my medical care.
I am no longer driving, and the case manager also assisted me in applying for transportation
services. When my identification documents had expired and /or were lost, the case manager
went through the maze of bureaucracy to renew and /or replace these documents.
In addition to all the services I listed above, the case manager has been a great emotional
support for seniors like me who live alone. I feel that I can always depend on the case manager
to provide great service should I have a care need arise. I entrust that the case manager would
always come to my aide and assist me the best she can.
The Case Management program is an extremely valuable service in Cupertino, and it helps
many seniors to continue to remain in their homes independently and safely. I am grateful that
the City of Cupertino is providing this quality program, and I support adding another full time
case manager to ensure more seniors are getting assistance.
