Reported to Devcon Construction at the jobsite as requested.
Checked welder qualification papers(Carlos Ramirez,ID#0982,Sean Massey,ID#7285). Checked WPSs and verified welding
parameters. Verified proper electrode storage. Verified proper preheat. Visually inspected joint fit-up. Performed continuous
visual inspection of the in-progress welding performed by two qualified welders using the FCAW process with E71T-8 wire
electrode.
Weldments consist of fillet welds joining HSS 4"x 4"x 1/4"to 5/8"x 6-3/4"Glu-lam plate and 3/4'x 12"base plate on lines 3 and 4 from
A to D at 111 level.
The preceding work was,to the best of my knowledge,in compliance with approved plans and specifications,including revisions
by engineer and Drawings S4,Details 2 and 1 and AB1 and E01,dated 5/26/15.

Reported to John Cortez of Lee's Imperial at the shop as requested.
Collected certified mill test reports and verified against material heat numbers. Checked welder qualification papers(Lucio
Ramirez,ID#3762,Gaspar Salazar,ID#8688,Saul Medina,ID#6456). Checked WPSs and verified welding parameters. Verified
proper electrode storage. Verified proper preheat. Visually inspected joint preparation and fit-up. Performed continuous visual
inspection of the in-progress welding performed by three qualified welders using the FCAW process with E71T-8 wire electrode.
Weldments consist of fillet welds and flare bevel groove welds joining 6"x 6"x 3/8"tube to A36 plates and tabs;and 8"x 6"x 3/8"
tube to A36 plates and tabs.
The preceding work was,to the best of my knowledge,in compliance with approved plans and specifications,including revisions
by engineer and shop drawing 05-15-028,#4,Details 4A2,4A1,4A3,4A4,4A5 and 4A6,dated 5/26/15.
