Permit to Operate. This permit will be issued upon the successful conclusion of our final construction
inspection. You will also be asked to submit a performance test of the mechanical exhaust

We have received drawings for the above project. We are unable to complete the review of the
drawings until the following information is provided:
1. Indicate the following storage areas on the revised drawing:
a. Dry food storage (minimum 96 linear feet of approved storage shelving)
b. Nonfood items, cleaners, towels and apparel
c. Soiled linens, towels, and apparel
d. Inside and outside garbage storage areas
e. Designated areas for mop buckets, brooms, dust pans and other cleaning items
2. Provide a detail showing that conduit of all types (electrical, water, and drain lines) will be
concealed within the wall and toe kick spaces of cabinets. A sample detail is enclosed with this
letter.
3. Include the phrase "smooth, no texture" and "light color" in the remarks column for both the wall
and ceiling surfaces on the room finish schedule. For the purposes of a food establishment, light-
colored means having a light reflectance value of 70% or greater.
4. Include the words "minimum 4-inch high 3/8-inch radius continuously cove base" in the base
remark column of the room finish schedule
5. Fill out and return, with the revised drawing, the enclosed Exhaust Ventilation System form. Be
sure to include one set of specification sheets with the performance curves for the blower(s),
filters(s) and make-up air unit(s).
6. Provide a sneeze guard detail showing a 54 - 60 inch nose/mouth zone to the top of the front
counter/food preparation display area.
7. Identify the location of the outside garbage storage area on the site plan.
8. Provide a copy of your food menu or a listing of all of the food and beverage items to be offered
at this food facility. Indicate if there will be any food processes that would involve the following:
a. Potentially hazardous foods
b. Foods that will be prepared or held for several hours prior to service
c. Foods requiring cooling and reheating
d. Multiple step processing (passing through the critical temperature zone, 135OF to 41OF more
than once)
e. Multiple ingredients being assembled or mixed
9. Verify that all food menu items come from an inspected and approved source.
10. Verify that there is adequate and approved freezer and refrigeration space available to store
frozen foods at 50F and below, and refrigerated foods at 41OF and below. Provide the method
used to calculate cold storage requirements.
