Re: Inspection and Construction Management Services for the Pumpkin Fiesta Storm Drain Project
CSG Consultants, Inc. (CSG) would like to thank the City of Cupertino (City) for the opportunity to present its
proposal to provide inspection and construction management services for the Pumpkin Fiesta Storm Drain
Project (Project).
S COPE OF WORK
We understand the project scope involves Replacement of various existing storm drain structures. This includes 36”
Reinforced Concrete Pipe, Box culverts with sizes of 4’x2’, 7’x3’, and 7’x4’, a pre-cast box culvert, and approximately
850’ of various cast-in-place structures. The Project duration is 90 working days, and the work is primarily in
residential areas.
The Project will be Awarded May 16, 2023 with an anticipated start date in mid-June. Due to the limited budget City
requested part-time inspection and construction management services.
PROPOSED STAFF
For this project, CSG proposes Nourdin Khayata as the main point of contact. His contact info is as follows:

Jim Mazzone will be assigned as the main inspector on this project. However, due to the timing and the part
time inspection requested by the City, CSG will assign other inspectors to help cover the work when Mr.
Mazzone is assigned to other projects. Mr. Khayata will be the CM through the project duration. Material
Testing is not part of this proposal.
COST
CSG stands ready to provide the requested services for this project. Provided below is the proposed fee.
