TO HUMAN SERVICE GRANT FUNDS
CITY/NON-PROFIT CONTRACT
The First Amendment to Contract is entered into between United Way of Silicon Valley
(CORPORATION) and the City of Cupertino
(CITY) to amend the Contract entered into pursuant to the allocation and disbursement of
Human Service Grant funds to Corporation dated Apr 15, 2008 (CONTRACT).
WHEREAS, the CITY has reserved a portion of i is General Fu>~to be used on activities that
benefit Cupertino's residents; and
WHEREAS, the City Council authorized the City Manager execute amendments with
organizations as needed to implement the FY 2009/10 allocation of Human Service Grant
funds; and
WHEREAS, the parties wish to enter into this amendment to extend the term of the
CONTRACT, revise the Project Description, Implementation Time Schedule and Project
Budget;
THEREFORE, the parties agree as follows:
1. Section I, entitled "Project," is revised to read as follows:
"CITY agrees to allocate a portion of its clzrrent Human Service Grant budget to
CORPORATION, being the sum of Two Thousand Dollars and No Cents ($2,000.00).
for the purpose of implementing the CORPORATION'S project, as more particularly
described in numerous exhibits as noted h~:rein, attached to this First Amendment.
2. Section II, entitled "Term," Parts A & B are revised to provide a contract end date of
June 30, 2010.
3. Supplement Exhibit A, the Project Description, Exhibit B, the Performance
Measurements, Exhibit C, Proposed Implementation Time Schedule, and Exhibit D,
Project Budget, with Revised Exhibits "A through D," attached hereto and
incorporated herein by this reference.
4. All terms, covenants and conditions stated in the CONTRACT, which are not herein
amended, remain in full force and effect.

2-1-1 Santa Clara County is anon-emergency, co~ifidential, three-digit phone number, authorized
by the California Public Utilities Commission (C]?UC) for use within the County of Santa Clara,
that provides streamlined access to critical health and human services information and referrals
at no fee to the caller. 2-1-1 Santa Clara County offers a wide selection of information and
referral services to address the many needs of community members:

2-1-1 SCC operates 24-hours a day, 7-days a week in English, Spanish, Vietnamese and more
than 170 other languages. Anyone can access 2-1-1 SCC anytime, whether through the call
center or via the public website (www.211SCC.oig). 2-1-1 SCC helps to eliminate the frustration
of making multiple calls to find the right service and updated contact information. 2-1-1 SCC
helps people navigate the health and human service system, whether in times of national or
regional crisis or in times of everyday needs and challenges. Data collected through 2-1-1 SCC

can be utilized for the improvement of services, as well as helping to identify gaps in service and
impact public policy. In times of wide-spread emergencies, 2-1-1 SCC becomes the logical
communications platform for the dissemination of information. 2-1-1 SCC enhances public
safety, crisis recovery efforts, and serves as a conduit for the dissemination of life-vital
information. We have seen the benefit of the 2-1-1 service in other areas of the country, most
recently during the time of the San Diego wildfires. 2-1-1 Santa Clara County, in participation
with the Santa Clara County Office of Emergency Preparedness, is integrated into the regional
disaster response plans.
2-1-1 SCC serves the entire City of Cupertino 24 hours a day. All Cupertino residents have:
• free access from cell phones, pay phones, home phones and all other land lines
• free access to the 2-1-1 SCC website (www.211SCC.org)
• free TTY access to 2-1-1 SCC by calling 866-390-6845.
