TO HUMAN SERVICE GRANT FUNDS
CITY/NON-PRO]?IT CONTRACT
The First Amendment to Contract is entered into between Community Technology Alliance
(CORPORATION) and the City of Cupertino
(CITY) to amend the Contract entered into pursuant to the allocation and disbursement of
Human Service Grant funds to Corporation dated. Apr 15, 2008 (CONTRACT).
WHEREAS, the CITY has reserved a portion of ::ts General Fun to be used on activities that
benefit Cupertino's residents; and
WHEREAS, the City Council authorized the Cite Manager execute amendments with
organizations as needed to implement the FY 20(19/10 allocation of Human Service Grant
funds; and
WHEREAS, the parties wish to enter into this amendment to extend the term of the
CONTRACT, revise the Project Description, Iml~lementatiou Time Schedule and Project
Budget;
THEREFORE, the parties agree as follows:
1. Section I, entitled "Project," is revised to :read as follows:
"CITY agrees to allocate a portion of its current Human Service Grant budget to
CORPORATION, being the sum of Two T"housand Dollars and No Cents ($2,000.00).
for the purpose of implementing the CORPORATION'S project, as snore particularly
described in numerous exhibits as noted herein, attached to this First Amendment.
2. Section II, entitled "Term," Parts A & B are revised to provide a contract end date of
June 30, 2010.
3. Supplement Exhibit A, the Project Description, Exhibit B, the Performance
Measurements, Exhibit C, Proposed Implt;mentation Time Schedule, and Exhibit D,
Project Budget, with Revised Exhibits "A through D," attached hereto and
incorporated herein by this reference.
4. All terms, covenants and conditions stated in the CONTRACT, which are not herein
amended, remain in full force and effect.

Project Description:
Please describe the project with as many details as possible. Include general information, such
as the number of units, number of Cupertino residents that will be served, services that will be
provided, etc.
The CDBG-eligible, shared web-based Homeless Management Information System increases the
effectiveness and efficiency of over 150 programs in Cupertino and Santa Clara County with a shared goal
of ending, preventing and shortening episodes of homel~:ssness. HMIS SCC serves as the primary client
data collection and management tool, benefits unhoused and at risk clients who receive snore holistic,
coordinated service and provides policy makers with an accurate count of those receiving services, helps
determine which services -and combination of services -work best. Of the 10,586 unhoused adults and
children receiving homeless services and shelter in the fiscal year ending June 30, 2009 who reported a
last permanent residence in Santa Clara County, 91 unduplicated individuals - 0.86% -- were from
Cupertino. Nine of these were children; three were seni~~rs; six were veterans; and seventeen were
chronically homeless. ~ Almost athird -twenty-nine -had no financial resources at program entry.
Community Technology Alliance was chosen by our County's Continuum of Care to operate the
HUD mandated program. In addition to providing, maintaining and enhancing the necessary technology,
Community Technology Alliance provides ongoing support and training to staff at 150+ partnering
programs that actively participate in HMIS. Plans for the funded year include working with these partners
(including West Valley Community Services) to ensure the depth, breath and quality of data, fostering
more holistic solutions to homelessness through interagency response and outreach to mainstream service
and benefits providers to ensure streamlined access and discourage discharging from County institutions
(including foster care, hospitals and jails) into hoineless~iess.
~ HUD defines the chronically homeless as disabled, single: adults with either four or more episodes of homelessness
in three years or an episode one year or longer.
