09-105 West Valley Community Services Human Services Grant FundsFIRST AMEI~iDMENT TO HUMAN SERVICE GRANT FUNDS CITY/NON-PROFIT CONTRACT The First Amendment to Contract is entered into between West Valley Community Services - Affordable Placement Program (CORPORATION) and the City of Cupertino (CITY) to amend the Contract entered into pursuant to the allocation and disbursement of Human Service Grant funds to Corporation dated E~Pr 15, 2008 (CONTRACT). WHEREAS, the CITY has reserved a portion of its General Fun~to be used on activities that benefit Cupertino's residents; and WHEREAS, the City Council authorized the City Manager execute amendments with organizations as needed to implement the FY 2005'/l 0 allocation of Human Service Grant funds; and WHEREAS, the parties wish to enter into this amendment to extend the term of the CONTRACT, revise the Project Description, Implementation Time Schedule and Project Budget; THEREFORE, the parties agree as follows: 1. Section I, entitled "Project," is revised to read as follows: "CITY agrees to allocate a portion of its current Human Service Grant budget to CORPORATION, being the sum of Seventy Five Thousand Dollars and No Cents ($75,000.00) for the purpose of implementing the CORPORATION'S project, as more particularly described in numerous exhibits as noted herein, attached to this First Amendment. 2. Section II, entitled "Term," Parts A & B are revised to provide a contract end date of June 30, 2010. 3. Supplement Exhibit A, the Project Description, Exhibit B, the Performance Measurements, Exhibit C, Proposed Implementation Time Schedule, and Exhibit D, Project Budget, with Revised Exhibits "A through D," attached hereto and incorporated herein by this reference. 4. Include general information, such as the number of units, number of Cupertino residents that will be served, services that will be provided, etc. West Valley Community Services (WVCS) has successfully implemented the Affordable Housing Program for the City of Cupertino for the past 15 years. The WVCS Affordable Housing Program administers the Cupertino Below Market Rate (BMR) Rental and Sales Programs. The BMR Rental Program places very low- to low-income residents in over 100 BMR rental units. WVCS documents eligibility and maintains an open working waiting list for the prospective tenants. WVCS re-qualifies all tenants on the anniversary of their move-in date or when the HUD guidelines are announced. WVCS also screens and places eligible seniors into affordable units identified b~- the City. Currently WVCS manages 10 senior units at Chateau Cupertino. The BMR Sales Program identifies, qualifies, and places median- and moderate-income homebuyers. WVCS qualifies and maintains a waiting list for future sales projects. WVCS also administers any resale of BMR purchased units to qualified buyers. WVCS staff conducts annual residency verification and audits. WVCS creates and distributes outreach materials to publici2;e the availability of all Affordable Housing programs. WVCS staff provides annual updated BMR information brochures and statistical information to the City and to the public. Staff works with the Housing Commission to ensure that the Program follows City goals and objectives on a continuous basis.
