Include general information, such as the number of units, number of Cupertino residents that will be served, services that will be provided, etc. The LIVE OAK adult day care center at 20920 McClellan Rd. will serve frail, dependent, low-income Cupertino seniors with a specialized program of recreation, mental stimulation, companionship and nutrition. In addition, the same number of family caregivers will have access to important respite and support services which will enhance their ability to maintain their senior loved ones living in the home environment. Services will target disabled Cupertino seniors with mobility and self-care limitations due to chronic a~e-related aisorders whicIi place them at high risk of social isolation and premature institutionalIzation. Typical conditions of program seniors include dementia, onset of Alzheimer's, Parkinson's Disease, stroke, cardiovas- cular disease and severe depression. The majority of senior clients reside in the cities of Cupertino and Sunnyvale. The day care program is structured to maximize the social experience of all participants while enhancing overall health through personal attention, mental stimulation, peer companionship and nutritious meals. Activities include arts and crafts, music, games, pet therapy, reminiscing, current events, seated exercise, birthday parties, etc. Sliding-scale fees (0-$52/day) make the program accessible and affordable for low-income and indigent clients. Families are encouraged to contribute what they can toward the cost of care for their senior participant, but most can afford only a small portion of the actual cost of service; several clients attend at no charge. No one is refused enrollment due to inability to pay. Adult day care gives much-needed respite to caregivers, reducing the stress associated with administering 24-hour care to dependent elders. Counseling, referrals and Caregiver Support Workshops help families maintain their dependent senior loved one in the home environment, avoiding the emotional and financial trauma of institutional placement. City of Cupertino General Funds will be allocated toward helping five Cupertino low-income seniors receive adult day care services at a significantly reduced rate of$5/day, and benefiting their caregivers with respite and support, as well as operating expenses to mamtain full enrollment, quality services, and adequate staff-to-client ratio at the Cupertino site.
