(Hereinafter referred as CONTRACTOR), in consideration of their mutual covenants, the parties hereto agree as

TERMS: The services and/or materials furnished under this Agreement shall commence on October 24, 2007
and shall be completed before November 30, 2007.
COMPENSATION: For the full perfonnance of this Agreement, CITY shall pay CONTRACTOR:
Not to Exceed of $4,806.00
Califomla Labor Coda, Section 1771 requires the payment 01 prevailing wages to all workers employed on a Public WOl1c$ contlaCt In excess of
$1.000.00.
GENERAL TERMS AND CONDITIONS
Hold Hanni.... Contractor agrees to save and hold harmIes8 the CIty, ils officers, agents and employees from any and all damage and liability due
to negligence, errors and omissions, inclUdng all costs of d9f8nd1ng any claim, caused by or arising out of the perfonnance of this Agreement CIty

Insurance. Shoulcl the CIty require evk:lence of Insurability, Contractor shall file with City a Certificate of Insurance before commencing any services
under this Agreement. Said Certificate shaH be subject to the approval of City's Director of Actninislrallve Services.
Non-Dlscrlmlnatlon. No dIscfImina1Ion shall be made in the employment of peI'SOflS under this Agreement becaUse of the race, color, natlonaI
origin, ancesIry, religion or sex of such per8OIl.
Interest of ContnlClor. It Is understood and agreed that this Agreement Is not a contract of employment In the sense that the relationship of master
and servant e)(j$1S between City and undersigned. At all tines, Contractor shall be deemed to be en independent contractor and Contractor Is not
authorized to bind the CIty to any oontracls or other obligationS in executing this agreement Contractor certifies that no one v.tJo has or will have
any fInenclallnt$f86t under this Agreement is an officer or employee of City.
Changee. ThIs Agreement 8hal1 not be assigned or transferred without the written COI18ent of the City. No changes or variations of any kind are
authorized Without the written oonsent of the City.

FoUowing is a quote to remove 1 piece of insulated glass with custom simulated sandblast pattern on the #3
surface (inboard lite) and replace it with a temporary unit We will then separate the broken unit, preserving the
custom sandblasted piece. OUf vendor will then fabricate a new insulated piece of glass to match existing using
the preserved piece and a new lite on the outboard side to match existing. SVG will retum the site and remove
the temporary glass then install the new unit Cost includes materials, labor and equipment necessary to
complete.
