For your review please find below a breakdown noting VAV boxes that we were able to repair/calibrate and confirm the operation, as well as those that require greater repairs. A.General notes: a)For those VAV boxes that we were able to confirm operation, with the use of a Magnehelic we also confirmed airflow min and max matched those listed on the provided As-Builts. b)The pneumatic systems operate with antiquated Siemens Dual-Tem thermostats, which are no longer readily available. We have ordered a few in anticipation of the repairs noted below and any extras will be kept on-site for future use. Please note that if OEM thermostats are no longer available, a retrofit of the pneumatic piping and controllers will be needed to accommodate any new model pneumatic thermostats. If this is the case, pricing for additional labor and materials will be provided. c)Some of the VAV boxes need blower assembly repairs/replacements; to access these assemblies the large VAV base panel needs to be disconnected. To do so we need to alter the fabric sealant in the discharge duct/flex connections; We have some asbestos concerns with this fabric sealant, see image below. To allow our team to move forward we need the Cupertino team to provide a report noting the absence of asbestos or allow our Health and Safety Director to test. Pricing for our safety team to test can be provided upon request. Exhibit A B. VAV boxes found to be operating properly and/or we repaired: TB1-2: The thermostat was found hanging from a ceiling tile; control air piping was also found disconnected at the volume controller. We mounted the thermostat on the wall and rerouted pneumatic piping to it. Calibrated the airflow, cycled the unit, and confirmed operation. TB1-6: This VAV was found out of calibration. Calibrated it and verified the min and max airflows and temperatures. TB1-11: Confirmed no thermostat servicing Dianne’s office. Traced pneumatic tubing and velocity controller, rerouted and altered the tubing to accommodate new thermostat. Calibrated and tested operation in heating/cooling- all satisfactory. TB1-13: Tested and Confirmed VAV box to be operating properly. TB1-17, TB1-18: These VAVs are operating properly. They are both controlled by one thermostat, which is not to code, but ok as they service similar perimeter zones. TB 1-19: Calibrated the airflow, cycled the unit, and confirmed operation. TB 1-20: Our tech found a defective damper actuator and replaced with new. Calibrated the airflow, cycled the unit, and confirmed operation. C. Pending Repairs: TB1-3: Our technicians confirmed the two blower motors to have shorted to ground and the relays to be worn (they were also found intermittently buzzing). Upon further examination he confirmed one of the motors to have collapsed. As a safety precaution, our tech cut power to the VAV box by Looking-Tagging-Out (LOTO) circuit breaker #30 and isolated the shorted wiring. After the repairs need to confirm the functionality of the pneumatic controls. The following scope is needed: • Remove ceiling tiles below the VAV box. • Disconnect the VAV base panel to allow for access. • Disconnect, remove and dispose of (x2) blower motor assemblies. • Furnish and install two new ¾ HP motors, blower wheels, and run capacitors. • Disconnect and replace coil contactor relay with new. • Start-up TB 1-3, test the pneumatic systems and note if additional repairs are required. • Reinstall the base panel on the VAV; reinstall the T-bar ceiling tiles. • Clean up the are of work. Our Price to complete these repairs is $7,995 TB1-5: Our tech found this box with a defective thermostat; the thermostat no longer bleeds off pressure to achieve the necessary communication PSI range. DESIGNATED CONSTRUCTION PROJECT(S) GENERAL AGGREGATE LIMIT This endorsement modifies insurance provided under the following: COMMERCIAL GENERAL LIABILITY COVERAGE PART SCHEDULE A.For all sums which the insured becomes legally obligated to pay as damages caused by "occur- rences" under Section I – Coverage A,and for all medical expenses caused by accidents under Section I – Coverage C,which can be attributed only to ongoing operations at a single designated construction project shown in the Schedule above: 1.A separate Designated Construction Project General Aggregate Limit applies to each des- ignated construction project, and that limit is equal to the amount of the General Aggregate Limit shown in the Declarations. 2.The Designated Construction Project General Aggregate Limit is the most we will pay for the sum of all damages under Coverage A,ex- cept damages because of "bodily injury" or "property damage" included in the "products- completed operations hazard", and for medi- cal expenses under Coverage C regardless of the number of: a.Insureds; b.Claims made or "suits" brought; or c.Persons or organizations making claims or bringing "suits". 3.Any payments made under Coverage A for damages or under Coverage C for medical expenses shall reduce the Designated Con- struction Project General Aggregate Limit for that designated construction project. Such payments shall not reduce the General Ag- gregate Limit shown in the Declarations nor shall they reduce any other Designated Con- struction Project General Aggregate Limit for any other designated construction project shown in the Schedule above. 4.The limits shown in the Declarations for Each Occurrence, Damage To Premises Rented To You and Medical Expense continue to apply. However, instead of being subject to the General Aggregate Limit shown in the Decla- rations, such limits will be subject to the appli- cable Designated Construction Project Gen- eral Aggregate Limit. Designated Construction Project(s): A GENERAL AGGREGATE LIMIT APPLIES TO EACH CONSTRUCTION PROJECT WHERE THE NAMED INSURED IS PERFORMING OPERATIONS, AND WHERE REQUIRED BY WRITTEN CONTRACT. HOWEV- ER, A GENERAL AGGREGATE LIMIT DOES NOT APPLY TO ANY CONSTRUCTION PROJECT WHERE THE NAMED INSURED IS PERFORMING OPERATIONS THAT ARE INSURED UNDER A WRAP UP OR ANY OTHER CONSOLIDATED OR SIMILAR INSURANCE PROGRAM. Information required to complete this Schedule, if not shown above, will be shown in the Declarations. B.For all sums which the insured becomes legally obligated to pay as damages caused by "occur- rences" under Section I – Coverage A,and for all medical expenses caused by accidents under Section I – Coverage C,which cannot be at- tributed only to ongoing operations at a single designated construction project shown in the Schedule above: 1.Any payments made under Coverage A for damages or under Coverage C for medical expenses shall reduce the amount available under the General Aggregate Limit or the Products-completed Operations Aggregate Limit, whichever is applicable; and 2.Such payments shall not reduce any Desig- nated Construction Project General Aggre- gate Limit. C.When coverage for liability arising out of the "products-completed operations hazard" is pro- vided, any payments for damages because of "bodily injury" or "property damage" included in the "products-completed operations hazard" will reduce the Products-completed Operations Ag- gregate Limit, and not reduce the General Ag- gregate Limit nor the Designated Construction Project General Aggregate Limit. D.If the applicable designated construction project has been abandoned, delayed, or abandoned and then restarted, or if the authorized contract- ing parties deviate from plans, blueprints, de- signs, specifications or timetables, the project will still be deemed to be the same construction pro- ject. E. The provisions of Section III – Limits Of Insur- ance not otherwise modified by this endorsement shall continue to apply as stipulated.
